I have a game host hosting my server, have installed the addon but seems to crash after about 3 or 4 maps, changed the * prefix to map names but still no luck, any help would be appreciated.

try using

OldRotation=1
//Enable cod4-style rotation
OldRotationVal=playlist terminal map mp_terminal_cls etc
//Rotation, change to your desire

Note:

you will need to create the map .dsr files
