Depends on the situation, the chance is not 100% and also not 0%. But I think like 1%.

Yes, but there aren't much hacks that are undetected + since there is a built-in report and theater mode in black ops, you are getting very soon busted on black ops. So watchout!

No, you may get banned for BO but that's all.
i dont think that the report option has any function at all.
you can use external esp, this is pretty safe
The report function is just some gay-ass bullshit that treyhard forgot to remove from the Eggsbawks version.
As for VAC just dont be stupid with it, the delayed bans are a bitch you dont know which hack is detected if you use more than one.
If you hack I recommend you to do it for 1 game long, not to much, because when people will find out they might will use theater mode to get you caught, also HC is harder for them to discover since it's no-killcam.
